Family Connects leaves an impact in North Carolina Counties

Family Connects, formerly Durham Connects, is the first home visiting model used for entire community. Results already show the positive impact of the program in Durham and the model has expanded to other communities such as Forsyth and Guilford Counties. Family Connects works by assigning a nurse to a family.  

This nurse will then visit with newborn parents, check the infant’s physical well-being, and ensure how the family is coping with the social and fiscal aspects of being a new parent. Through home visits, nurses assess the needs of parents and link parents who need assistance with community resources. Examples of community resources nurses connect families to include parenting classes, breastfeeding assistance, and assistance finding childcare.
